Family Navigation Network

情色五月天’s Family to Family Health Information Center for families who have children & youth with special health care needs.

What are you looking for?

No 情色五月天 family should have to feel alone. Ever.

Caring for children with special needs and disabilities can be overwhelming, exhausting and complex. Often families and caregivers struggle to find the resources and services their child and family may need.

The Family Navigation Network (FNN) is a statewide project of the 情色五月天 Center for Excellence in Disabilities providing information, support, training and resources at no cost to assist families caring for children and youth with special needs. We are staffed by parents and family members of children with special health care needs and disabilities, so we know the journey.

We promote family-centered care and family and professional collaboration at all levels of health care. The FNN also has a strong commitment to promote and support the needs of families from racially, culturally, and linguistically diverse communities.

“You deserve some type of major award just based on the amount of care and patience that you have for panicking moms... I'm sure I'm not the only one. Sending you a GIGANTIC virtual hug!!!”
